At this presentation, you’ll learn what Advance Care Planning (ACP) is, how to have an ACP conversation with loved ones, steps to create an Advance Directive (AD), how to review an existing AD, and how to choose the right person (Patient Advocate) to speak for you when you cannot speak for yourself.
Carol Robinson DNP, RN, Community Coordinator of Making Choices Michigan, will lead this interactive presentation. Carol helps you think and talk about your values, beliefs and preferences for end of life care and for care anytime you might not be able to speak for yourself! And what if you’ve been asked to speak for someone else – are confident you know that person’s wishes? Carol will also discuss the value of using a trained facilitator to guide conversations about wishes for future medical care.
