As the Hope Church community, we continue to look to the future. Led by the Discerning Our Future Committee, Consistory is repeating an information-gathering process we began in 2015. To help us gain a clear picture of where we are now and what parts of our life together may need examination, we ask that Hope Church members and friends complete an online survey. Please read these instructions before beginning.

Your responses are completely confidential and will not be identifiable to anyone at any time, before, during or after this survey. The survey is available through this link: https://www.assessmypolarities.com/awU200eZ.​ We ask that you respond by November 2.

Please respond honestly to each statement in the survey. The survey consists of 48 statements; please rate the frequency with which you experience or observe the situation or behaviors. You may see some behaviors you never experience! As you consider your answer for each statement it is very important that you keep in mind the framing question: “Based on what I see and experience, I’d say…”

The rating scale for your answers is:
Almost Never | Seldom | Sometimes | Often | Almost Always

Some tips for taking the survey:

  • Select your demographics using the pull-down menus.
  • Answer each question using the five radio buttons.
  • Try to use “Sometimes” as little as possible.
  • Leave a statement unanswered if you have no observation or experience.
  • You may add a comment about any statement by clicking a quote balloon.
  • To log out and return later, click the Save & Exit button at the bottom of the page.
  • When you’re done, click the Submit Survey button at the bottom of the page

A paper copy of the survey is available upon request from the church office. You can also contact the church office for assistance with completing the survey.

Results of the survey will be shared with Consistory and with the congregation in the new year.
